General Provisions

  1. These Regulations define the rules and conditions for conducting and participating in the training cycle titled 'Projekt Tkanki Miękkie', which will consist of 8 theoretical and practical modules ('Course').
  2. The provisions of the Regulations constitute an integral part of the application for participation in the Course and apply to all Participants.
  3. Detailed information about the Course, including the dates of individual modules and their program, is included in the Course Information Form, which is an integral part of the Regulations. The prices listed in the Regulations and the Course Information Form are gross prices.
  4. The organizers of the course are: Agnieszka Peret, conducting business under the name PERIOMED Agnieszka Peret, ul. Starej Gruszy 8/9, 03-289 Warsaw, and Bartłomiej Górski, conducting business under the name Bartłomiej Górski, al. Jana Pawła II 67/32, 01-038 Warsaw, jointly conducting business under the name 'Agnieszka Peret Bartłomiej Górski' civil partnership, address ul. Starej Gruszy 8/9, 03-289 Warsaw, NIP 5242939491, email: projekttkankimiekkie@gmail.com ('Organizer').

Course Participation Rules and Fees

  • A participant in the Course can be a person with a medical education.
  • Conditions for participating in the Course are:
  • submitting an application for participation in the Course by sending an email to the Organizer's email address and paying the full Course fee to the bank account specified by the Organizer: 05 1140 2004 0000 3102 8239 7585 (mBank) - the date of crediting the Organizer's bank account is decisive, within the following deadlines:
    1. for a person in a postgraduate internship as a dentist or in the course of dental specialization - the Course fee is 41,000.00 PLN (forty-one thousand) and is payable according to the following schedule: First installment of 14,000.00 PLN (fourteen thousand) on the day of submitting the application for the Course, Second installment of 14,000.00 PLN (fourteen thousand) before the start of the 5th module of the Course, Third installment of 13,000.00 PLN (thirteen thousand) before the start of the 8th module of the Course.
    2. for other participants - the Course fee is 44,000.00 PLN (forty-four thousand) per person and is payable according to the following schedule: First installment of 15,000.00 PLN (fifteen thousand) on the day of submitting the application for the Course, Second installment of 15,000.00 PLN (fifteen thousand) before the start of the 5th module of the Course, Third installment of 14,000.00 PLN (fourteen thousand) before the start of the 8th module of the Course.
    3. in the case of simultaneous registration of two people, the Course fee is 42,000.00 PLN (forty-two thousand) for each person and is payable according to the following schedule: First installment of 14,000.00 PLN (fourteen thousand) on the day of submitting the application for the Course, Second installment of 14,000.00 PLN (fourteen thousand) before the start of the 5th module of the Course, Third installment of 14,000.00 PLN (fourteen thousand) before the start of the 8th module of the Course.

  • email confirmation of participation in the Course sent by the Organizer to the email address from which the application was submitted.
  • The Organizer's confirmation of participation in the Course constitutes the conclusion of an agreement for participation in the Course between the person qualified for the Course ('Participant'), and the Organizer.
  • The Organizer will issue an invoice (non-VAT) for participation in the Course within 7 business days from the date of confirmation of participation and send it electronically to the email address used for the application.
  • Participants bear the costs of travel and accommodation at the location of the Course. The Organizer will provide materials and preparations appropriate to the Course program, as well as protective clothing and meals during coffee breaks.
  • Participants who complete all modules of the Course will graduate. The Organizer may provide a knowledge test acquired during the Course, the completion of which will be a condition for receiving a certificate of Course completion.
  • Completion of the Course by the Participant will be confirmed by a certificate issued by the Organizer. The certificate will include, among other things, the Participant's name, Course title, number of Course hours, and the name of the Organizer.
  • The Organizer ensures that all preparations used during the Course come from legal sources, and their import, use, and disposal comply with legal regulations. During the use of preparations, Participants are required to exercise due caution and follow the Organizer's recommendations.

Rules for Withdrawal from the Course

  1. Participants may withdraw from the Course by sending a resignation statement to the Organizer's email address.
  2. In case of withdrawal from the Course 60 (sixty) days before its start, Participants will receive a refund of the Course fee paid, reduced by a cancellation fee of 500 PLN (five hundred).
  3. In case of withdrawal 30 (thirty) days before the start of the Course, Participants will receive a refund reduced by 30% of the total Course fee they would have paid if they had not withdrawn.
  4. If the Organizer receives a withdrawal notice after the start of the first module or if Participants do not inform the Organizer about their withdrawal, the Organizer will retain the entire Course fee as a cancellation fee.

Organizational Changes and Course Cancellation

  1. The Course will take place in-person at a hotel conference center in Warsaw. The exact location will be provided to Participants after their participation is confirmed by the Organizer.
  2. The Organizer reserves the right to change the dates of individual Course modules or cancel the Course or individual modules due to insufficient interest or other factors beyond the Organizer's control.
  3. If, due to unforeseen circumstances, a Participant cannot attend a module, the Organizer will provide the Participant with educational materials included in the module's program.
  4. If the Course is canceled for reasons beyond the Organizer's control, Participants will be refunded 1/8 (one-eighth) of the total Course fee paid for each canceled module, reduced by costs incurred by the Organizer.
  5. In the event of cancellation or rescheduling of individual modules, the Organizer is not responsible for costs incurred by Participants related to travel and accommodation unless the cancellation or rescheduling was solely due to the Organizer's fault, and Participants were informed later than 2 (two) business days before the planned start of the canceled module.
  6. The Organizer reserves the right to make changes to the Course program, such as rescheduling module dates or locations, while guaranteeing that all modules will take place in-person at a hotel conference center in Warsaw, subject to item 19 above.

Liability

  • The Organizer undertakes to conduct the Course with due diligence, considering the professional and specialized nature of its activities.
  • Participants bear full financial responsibility for damages caused by them.
  • The Organizer is not responsible for items lost, destroyed, or stolen during the Course.
  • The Organizer is not liable for damages incurred by Participants or third parties as a result of or in connection with the use of information, knowledge, or skills acquired during the Course in a manner inconsistent with their intended purpose.

Complaints

  • All complaints regarding the Course should be submitted via email to the Organizer's email address.
  • Participant complaints may be submitted no later than 14 days after the end of the Course. Complaints submitted later will not be considered.

Image and Sound Recording

  • The Organizer allows the recording of the Course using recording devices. It is forbidden to use materials obtained by Participants during the Course from the Organizer or lecturer for commercial purposes, especially for conducting other courses without prior written consent from the Organizer and the lecturer. The content of the Course and the materials provided to Participants during the Course constitute works within the meaning of the Act of February 4, 1994, on copyright and related rights (consolidated text Journal of Laws of 2021, item 1062, as amended) and are protected by law.
  • The Organizer reserves the right to record the event (take photos, record videos) for informational and promotional purposes related to its activities. For profile photos, individuals whose data is concerned, will be asked for their voluntary consent to the processing of their personal data and its publication.

Final Provisions

  • Submitting an application for participation in the Course by sending an email to the Organizer's email address constitutes acceptance of the provisions of the Regulations and other arrangements made between the Participant and the Organizer.
  • Non-compliance with the Regulations may result in the Participant's exclusion from the Course. In such a case, the Participant is not entitled to a refund of the Course fee.
  • Matters not regulated by the provisions of the Regulations are subject to generally applicable provisions of Polish law.
  • Any disputes arising from participation in the Course will be resolved by the court competent for the location of the Course.
  • The Organizer's liability to the Participant, regardless of its legal basis, is limited to the amount of the Course fee paid by the Participant. The Organizer is only liable for typical and actual damages, excluding lost profits. The Organizer is not liable for damages or unfulfilled obligations caused by force majeure or any reasons beyond its control.
  • The integral parts of the Regulations are: the Course Information Form and the Information Clause.

Purpose and Legal Basis for Processing Personal Data

Your personal data will be processed exclusively for the purpose of:

  • performing actions leading to the conclusion of an agreement or fulfilling the provisions of an agreement, in accordance with Article 6(1)(b) of the GDPR,
  • fulfilling legal obligations incumbent on us, in accordance with Article 6(1)(c) of the GDPR, arising from tax regulations - archiving documents related to the agreement and the Civil Code,
  • pursuing our legitimate interests, in accordance with Article 6(1)(f) of the GDPR, such as potential claims or defense against claims, ensuring the highest quality of service, and contacting you regarding organizational matters related to the Course,
  • sending commercial information, processing data for a waiting list or registering and publishing your image if consent is given, in accordance with Article 6(1)(a) of the GDPR.

Recipients of Data

The recipients of your personal data may include the Tax Office, postal service providers, a law firm assisting us in legal proceedings in the event of claims, and entities cooperating with us in connection with the provision of services:

  • hosting services,
  • delivery of IT solutions, software, IT support, and website positioning,
  • accounting office.

Data Retention Period

Your personal data will be processed for the following periods:

  • 5 years (from the end of the year) for data processing regulated by legal provisions, counted from the date of the end of cooperation, in accordance with Article 74 of the Accounting Act - in the case of individuals participating in the Course,
  • until consent for data processing is withdrawn, for data processing based on consent,
  • until claims expire or until a valid objection is submitted.

Rights

In connection with the processing of personal data, you have the following rights:

  • the right to access your data,
  • the right to rectify data,
  • the right to delete data if processed without a legal basis,
  • the right to transfer data based on consent or a contract,
  • the right to restrict processing,
  • the right to object,
  • the right to withdraw consent at any time. Withdrawal of consent does not affect the lawfulness of processing based on consent before its withdrawal,
  • the right to lodge a complaint with the Polish Data Protection Authority (UODO) if you believe that the processing of your personal data violates GDPR.

Voluntariness of Providing Data

Providing your data is voluntary. Providing personal data is a condition for participating in the Course. Failure to provide personal data will result in the inability to conclude a participation agreement.
